To make your visit smoother and reduce administrative steps, we require a secure card on file.
Here’s how it works:
At check-in:
Your preferred payment method is securely stored (including HSA/FSA cards)
After your insurance processes your claim:
You’ll receive an email outlining any remaining balance
The balance will be automatically charged to your card on file
A receipt will be emailed to you for your records
Your information is securely processed through our PCI-DSS compliant payment partner and is never stored on our internal systems.

Missed Appointment: $50
Missed Physical: $100
Late Cancellation: $50
Missed Procedure: $200
FOR PATIENTS WITH OUT-OF-NETWORK COVERAGE
If your insurance is out-of-network, the following applies:
New Patients: Full charge or minimum $200 deposit
Established Patients: Full charge or minimum $150 deposit
for patients paying directly for care
New Patients: Full charge or minimum $200 deposit
Established Patients: Full charge or minimum $150 deposit
Procedures: Full charge or minimum $200 deposit
